Individual Membership

Affiliate - State public health associations requesting affiliation and approved by the Governing Council and who pay dues annually. Annual dues: $0.75 per affiliate member up to 1,000 - over 1,000 affiliate members $750.00 plus $0.60 per member over 1,000.

Individual- Persons interested in the Association and pay $25.00 annually; $20.00 for first time new members.

Fellow - Individuals who are interested in the Association, who apply for and are elected to fellowship by the SHA Governing Council, and pay annual fellowship dues of $40.00.

Student - Full time students in public health or related field and pay $10.00 annually.

Retiree - Persons who are retired from public health or related fields may remain members by paying $10.00 annual dues.

Contributing - Companies who have an interest in public health - Annual dues $300.00.

Associate - Agencies and education institutions who have an interest in public health - Annual dues $150.00.

Individuals interested in individual, fellow, student or retiree membership need to complete the accompanying membership application and return with appropriate annual dues to: Southern Health Association, 1590 North Bass Drive, Mt. Juliet, TN 37122. Only fellow, individual or retiree members may vote or hold office in SHA.
